Nurse-Family Partnership helps transform the lives of vulnerable first-time moms and their babies. Through ongoing home visits from registered nurses, low-income, first-time moms receive care and support they need to have a healthy pregnancy, provide responsible and competent care for their children and become more economically self-sufficient. From pregnancy until the child turns two years old, Nurse-Family Partnership Nurse Home Visitors form a much-needed trusting relationship with the first-time moms, instilling confidence and empowering them to achieve a better life for their children - and themselves!

An evidence-based community health program, Nurse-Family Partnership's outcomes include long-term family improvements in health, education, and economic self-suffciency. By helping to break the cycle of poverty, we pay an important role in helping to improve the lives of society's most vulnerable members, build stronger communities, and leave a positive impact on this and future generations.

Why nurses?


For a young woman, a nurse is a trusted, respected, and accessible figure. a nurse bring specialized healthcare knowledge and can spend time with her moms. She approaches them without value judgments or a personal agenda. As the research shows, this relationship of trust makes a measurable difference for the whole family.

Goals

Improve pregnancy outcomes by helping women engage in good preventive health practices, including obtaining thorough prenatal care from their health care providers, improving their diet, and reducing their use of cigarettes, alcohol and illegal substances.

Improve child health and development by helping parents provide responsible and competent care for their children.

Improve the economic self-sufficiency of the family by helping parents develop a vision for their own future, plan future pregnancies, continue their education, and find work.

Services Provided

Nurses help moms:
• Have a healthy pregnancy and a healthy baby
• Become a better parent
• Build a strong network of support for mom and baby
• Make the home a safe place for the baby to live and play
• Get referrals for health care, child care, job training and other services
• Find ways to continue education and develop job skills
• Set goals for the family's future

Eligibility

Any woman who:
• Is less than 28 weeks pregnant with her first child
• Meets income requirements (eligible for WIC or Medi-Cal)
• Solano County resident

Moms are encouraged to enroll as early as possibly in their pregnancy!

How to Enroll

For first time pregnant moms: Join the program by calling 1-877-680-2229 (BABY).

For community agencies and providers:
Complete the
MCAH Central Referral Form, select NFP under "Programs," and fax to 707-784-2229 (BABY).
